DADS to Release Relocation Services RFP

DADS anticipates releasing a Request for Proposals (RFP) for “Relocation Services for Nursing Facility Residents” in the next two weeks. This announcement is made to allow potentially interested vendors time to prepare their responses and, if interested, partner with other entities.

Entities such as centers for independent living, area agencies on aging, aging and disability resource centers (ADRCs), and other organizations that work with individuals who are older or have a disability may consider responding to the RFP.

HHSC will post all official communications regarding this procurement on the Electronic State Business Daily (ESBD) website located at: Interested parties should check the ESBD frequently for notice of matters affecting the procurement.

As part of DADS Promoting Independence Initiative, Relocation Services contractors provide education and outreach activities to nursing facility residents. The purpose of the outreach is to increase awareness of community living options and supports and to inform residents of the relocation services that may be available to assist them in transitioning to the community. For those individuals who indicate an interest in relocating to the community, Relocation Services contractors perform identification, assessment and referral functions and for Medicaid eligible individuals, provide relocation assistance such as arranging transportation, assistance with locating and establishing housing, and other activities that will enable an individual to successfully relocate to the community. Coordination with Local Intellectual and Developmental Disability Authorities (LIDDA), Managed Care Organizations and other community-based service providers involved is required.
