How can I report a complaint regarding a hospital, dialysis center, abortion center, outpatient center, birthing center or rehabilitation facility?

How can I report a complaint regarding a physician, physician assistant, acupuncturist?
Call 1-800-201-9353 or click here to file a complaint online. The Texas Medical Board licenses physicians, physician assistants and acupuncturists.
How can I report a complaint about a long-term care insurance company?

How can I report a claim of discrimination?
Call 1-888-388-6332, TDD 1-877-432-7232, if you think you have been discriminated against by a Texas health and human services employee or contractor. These complaints are handled by the Texas Health and Human Services Commission Civil Rights Office.
