State Center Satisfaction Survey — Family/LARs
The state supported living centers and Rio Grande State Center are striving to improve the quality of care of those we serve. These efforts include gathering feedback from family members, legally authorized representatives (LARs) and correspondents. Every month, 8.3 percent of the center’s population will be randomly selected and asked to complete the "State Center Satisfaction Survey — Family/LARs." Your responses will help the facility determine your level of satisfaction with the state center services provided both to the individuals served, and to you in your role as a family member, LAR or correspondent.
If you wish to complete a paper survey, please request the survey instrument from your Center.
Those who complete the survey will remain anonymous. However, at the end of the survey, you may choose to share your contact information if you wish to be contacted for follow up on your concerns.
Please respond to each of the questions within the survey. Your feedback will help us improve the quality of services we provide to each individual at our facilities.
Please select “N/A” (not applicable) when a particular question does not affect the individual being served.
