How to use this siteThe Minimum Data Set (MDS) is a standardized collection of demographic and clinical information that describes a person's overall condition. All licensed nursing facilities in Texas are required to submit MDS assessments for all residents admitted into their facility.
More basic information about the MDS and data confidentiality can be found in the Introduction to MDS. New facilities and new MDS coordinators should refer to the MDS Steps to Success for a comprehensive explanation of the MDS preparation and submission process.
