Please remember to submit complete CWP reassessment packets to the assigned case manager by the end of the calendar month that ends two months before the expiration date of the CWP consumer's individual service plan. Please refer to Appendix VII of the CWP Provider Manual for specific reassessment packet due dates that participant's ISP.
